In some young children brought by their parents for diagnosis of acute life-threatening events investigations suggested imposed apnoea as the cause rather than spontaneous occurrence. Covert video surveillance of the cot in which the baby was monitored allowed confirmation or rebuttal of this diagnosis. That parents were not informed of the video recording was essential for diagnosis and we assert ethically justifiable as the child was the patient to whom a predominant duty of care was owed. When the editor invites comment on a response to an analysis of a criticism of a protocol already defended by the author in this journal, the issue is clearly contentious. I will comment briefly on Thomas's paper in this issue of the journal, and look at points of agreement as well as dissent.

Aggleton & Brown argue that a hippocampal-anterior thalamic system supports the “recollection” of contextual information about previous events, and that a separate perirhinal-medial dorsal thalamic system supports detection of stimulus “familiarity.” Although there is a growing body of human literature that is in agreement with these claims, when recollection and familiarity have been examined in amnesics using the process dissociation or the remember/know procedures, the results do not seem to provide consistent support. We reexamine these studies and describe the results (...) of an additional experiment using a receiver operating characteristic (ROC) technique. The results of the reanalysis and the ROC experiment are consistent with Aggleton & Brown's proposal. Patients with damage to both regions exhibit severe deficits in recollection and smaller, but consistent, deficits in familiarity. (shrink)

Background The consistency of codes governing disclosure of terminal illness to patients and families in Islamic countries has not been studied until now. Objectives To review available codes on disclosure of terminal illness in Islamic countries. Data source and extraction Data were extracted through searches on Google and PubMed. Codes related to disclosure of terminal illness to patients or families were abstracted, and then classified independently by the three authors. Data synthesis Codes for 14 Islamic countries were located. Five codes (...) were silent regarding informing the patient, seven allowed concealment, one mandated disclosure and one prohibited disclosure. Five codes were silent regarding informing the family, four allowed disclosure and five mandated/recommended disclosure. The Islamic Organization for Medical Sciences code was silent on both issues. Conclusion Codes regarding disclosure of terminal illness to patients and families differed markedly among Islamic countries. They were silent in one-third of the codes, and tended to favour a paternalistic/utilitarian, family-centred approach over an autonomous, patient-centred approach. (shrink)

The Islamic Medieval Philosopher al-Ghazᾱlî, known to the Latins as Algazel, was influential in the shaping of the intellectual philosophic movements in the thirteenth century. Though Ghazali’s predecessor Ibn Sînᾱ and successor Ibn Rushd received the philosophic credit due to them, Ghazali’s own philosophic ideas have not been significantly assessed; and hence Ghazali’s “fame” lies, we are told, in being responsible for the decline of Medieval Philosophy, especially Islamic Philosophy, a claim that is extremely difficult to prove. But be that (...) as it may, the intellectual movement in Europe during the thirteenth century pursued the intellectual Islamic heritage, not just of Avicenna and Averroës, but also that of Ghazali. Granted that Ghazali’s philosophic ideas did not assume a definite pattern in the philosophic literature—as was the case with Avicenna and Averroës—still a careful study of Ghazali’s works will reveal how profound and widespread his influence was on Western Medieval scholars. A case in point is the influence of Ghazali on Saint Thomas Aquinas—who studied the works of the Islamic philosophers, especially Ghazali’s, at The University of Naples. Thus in the course of the subsequent analysis, similarities between Ghazali and Aquinas will reveal themselves. The aim of this paper, however, does not consist in delineating their refutations of the arguments of the philosophers that one finds in Ghazali’s and Aquinas’s works. Rather I shall primarily deal with their discussion of the principle of causality and then indicate how the works of Ghazali have played an important role in the shaping of the philosophic ideas of Aquinas. Standard models of adolescent risk taking posit that the cognitive abilities of adolescents and adults are equivalent, and that increases in risk taking that occur during adolescence are the result of socio emotional differences in impulsivity, sensation seeking, and lack of self-control. Fuzzy-trace theory incorporates these socio emotional differences. However, it predicts that there are also cognitive differences between adolescents and adults, specifically that there are developmental increases in gist-based intuition that reflects understanding. Gist understanding, as opposed to verbatim-based analysis, (...) generally has been hypothesized to have a protective effect on risk taking in adolescence. Gist understanding is also an essential element of informed consent regarding risks in medical decision- making. Evidence thus supports the argument that adolescents’ status as mature minors should be treated as an exception rather than a presumption, because accuracy in verbatim analysis is not mature gist understanding. Use of the exception should be accompanied by medical experts’ input on the bottom-line gist of risks involved in treatment. (shrink)

In an article entitled ‘Some Facts about our Oldest Latin Manuscripts,’ which appeared in this journal, the present writer put together a group of forty-seven MSS. which had this one feature in common, that each page or each column of a page they contained began with a large letter, regardless of whether that letter occurred in the middle of a sentence, or even in the middle of a word. It so happened that the list thus drawn up was composed almost (...) entirely of very ancient MSS.; indeed, it contained a very large proportion of the oldest MSS. extant. In the circumstances it seemed useful to note down the behaviour of these MSS. with respect to four other features—namely, the manner of indicating the omission of m and n at the end of lines, the use of running titles in the top margin, the size and arrangement of the written space, and the manner of signing quires. Although students of the fifth century A.D. have not been slow to recognize the merits of the ͉στορα Βυζαντιακ of Priscus, few efforts seem to have been made to under-stand this historian's methods of composition. The purpose of the present note is to indicate that the literary fashions of his time have exercised an unfortunate influence on at least one part of Priscus' work.
